ID Consults Roster # 1

koli-bacteria-123081_640I have decided to keep an ID consults roster. Essentially a log of the inpatient consultations I have seen during my infectious diseases fellowship training. Here, I will break them down by diagnosis. It is possible for one patient to have several infectious diseases diagnoses. The number after the diagnosis indicates unique patients. For example, I saw two different patients with salmonellosis!

During my first month as the ID fellow on the wards I saw about 65 new patients. It was busy!

1. BIODEFENSE

  • none

2. BONE/JOINT

  • Left heel osteomyelitis
  • Pseudomonas aeruginosa spinal osteomyelitis/hardware infection
  • Methicillin resistant Staph aureus right shoulder septic arthritis, community acquired
  • Methicillin resistant Staph aureus T7 – T8 osteomyelitis
  • Staph aureus hip osteomyelitis
  • Methicillin resistant Staph aureus ulnar osteomyelitis
  • Tuberculous osteomyelitis (Pott’s disease)
  • Group B strep and coagulase negative staph diabetic foot ulcer, ? osteomyelitis
  • Staph aureus L4 – S1 osteomyelitis
  • Extended spectrum beta-lactamase producing (ESBL) Klebisiella pneumoniae diabetic foot infection/osteomyelitis
  • T4 – T11 osteomyelitis
  • Humeral osteomyelitis, hardware associated

3. CARDIAC

  • Presumed methicillin resistant Staph aureus endocarditis
  • Staph aureus tricuspid valve endocarditis
  • Staph aureus endocarditis

4. DERMATOLOGIC

  • Staph aureus and Group B strep furuncle
  • Salmonella cellulitis with abscess
  • Presumed Vibrio vulnificus cellulitis
  • Insect bite associated cellulitis
  • Intravenous drug abuse related hand cellulitis
  • Methicillin resistant staph aureus hand cellulitis with abscess
  • Psychogenic skin picking with bacterial super-infection
  • Staph aureus cellulitis with abscess
  • Enterococcus decubitus ulcer

5. EYES/ORBITS

  • Pre-orbital cellulitis

6. FEVER

  • Neutropenic fever
  • Fever of unknown origin

7. GASTROINTESTINAL

  • Clostridium difficile colitis (6)
  • Antibiotic associated diarrhea
  • Escherichia coli liver abscess
  • Acute appendicitis
  • Pseudomonas aeruginosa abdominal abscess
  • Candida albicans & Candida glabrata abdominal abscess
  • Escherichia coli & Candida albicans peritonitis
  • Spontaneous bacterial peritonitis
  • Acute pancreatitis (2)
  • Salmonellosis (2)
  • Sigmoid diverticulitis with abscess

8. GENERALIZED INFECTION

  • none

9. GENITOURINARY

  • Pseudomonas aeruginosa urinary tract infection (2)
  • Vancomycin resistant enterococcus urinary tract infection
  • Candida albicans urinary tract infection
  • Escherichia coli urinary tract infection

10. GYNECOLOGIC

  • none

11. HEENT

  • none

12. HIV/AIDS

  • Pneumocystis jiroveci pneumonia (3)
  • Oral-esophageal candidiasis (3)
  • Cryptococcemia
  • Disseminated Mycobacterium avium
  • Wasting syndrome
  • Chronic diarrhea
  • Toxoplasmosis
  • Cytomegalovirus retinitis

13. NEUROLOGIC

  • Tuberculous meningitis
  • Staph aureus L4 – S1 epidural abscess
  • Aseptic meningitis
  • T4 – T11 epidural abscess

14. RESPIRATORY

  • Methicillin resistant Staph aureus ventilator associated pneumonia (3)
  • Multi-drug resistant Acinetobacter baumanii ventilator associated pneumonia
  • Streptococcus pneumoniae multi-lobar necrotizing pneumonia, community acquired in HIV patient
  • Multi-drug resistant Pseudomonas aeruginosa ventilator associated pneumonia (2)
  • Serratia marescens and Staph aureus ventilator associated pneumonia

15. SEPSIS SYNDROMES

  • Candida glabrata fungemia
  • Strep viridans bacteremia
  • Acinetobactar baumanii vascular catheter infection
  • Lactobacillus bacteremia, recurrent
  • Methicillin resistant staph aureus bacteremia (2)
  • Staph aureus bacteremia (2)
  • Coagulase negative staphyloccocal bacteremia
  • Candida albicans fungemia
  • Candida dublinensis fungemia
  • Pantoea agglomerans bacteremia

16. SEXUALLY TRANSMITTED INFECTIONS

  • Primary syphilis
  • Presumed neurosyphilis

17. SURGICAL INFECTIONS

  • Surgical wound infections

18. TRAVEL/TROPICAL

  • Dengue fever
  • Malaria, presumed Plasmodium falciparum

19. VACCINE ADVERSE REACTIONS

  • none
